    PRESCHOOL (Mad River School District Residents Only) 

    • Preschool Applications for 2024-25 are now available.
    • Applications may be picked up at the Mad River Student Services Office located at 801 Old Harshman Rd. 
    • Applications are also available for download at Preschool Registration.   
    • Preschool registration must be completed/returned to the Mad River Student Services Office/Board of Education, not via Oneview. 
    • Open Enrollment is not available for Preschool.

     K through 12th Grade REGISTRATION

    *Do you live outside of Mad River School District? If Yes, the application period is now CLOSED FOR 24-25 SCHOOL YEAR

    Registration Process

    • 24-25 Online Registration is open.
    • Approved (New) Open Enrollment families will be notified separately of the registration process. (Open Enrollment - Closed for 2024-25)
    • Submit an online registration form by clicking "REGISTER A NEW STUDENT" below to create/access your Parent Portal Account.
    • If you do not have a parent account, you will need to create one by selecting “Create New Account” inside the Oneview registration portal.
      • If you already have Oneview account, you will log in with your existing parent log-in information and select "Enroll New Student."
    • Grades K-4 - You must submit the online application for the building your student will attend based on your street address. School Finder - Street Listing   
      • If you are interested in an Intra-district Transfer request, please speak with the building secretary during your appointment time.
      • K-4 requires an online registration appointment to be scheduled.  You will need to click on your student's building website below and select "Registration Appointment."
    • Grades 5-12 - You will need to contact the building once you have submitted your online application to discuss that building's specific enrollment process. 
    • If you do not have the ability to complete the process online, please contact the building you wish to enroll in for assistance.

    Registration Documents - REQUIRED at Registration Appointment

    • Child’s certified birth certificate issued by the State's Health Department/Dept of Vital Statistics (*Hospital/Souvenir Birth Record will not be accepted*)
      • If the Birth Certificate is in any language other than English, applicants must provide a certified, translated copy with the original.
      1. The first must be a current lease agreement, mortgage statement/ deed, or rental agreement with the legal/custodial parent's name and address listed on the document.  Tax documents will not be accepted. **If you reside with someone else or there is not a lease or rental agreement, please contact your student's building for more information regarding additional residency document requirements.
      2. The second can be a current utility bill, cable/internet bill, homeowner/renter insurance, vehicle registration, payroll stub
    • Copy of Current IEP/ETR, Service Plan, if applicable
    • Copy of 504 Plan, if applicable
    • Copy of Gifted information, if applicable
    • Report Card from the previous school attended, if applicable

    School Assignment
    For Grades K through 4, students are assigned a neighborhood school based on their home address.  School Finder - Street Listing

    Intra-District Transfer Request School Transfers
    Transfers to another elementary within the District must be made through an intra-district request. Please contact your student's assigned elementary school based on your address or by calling the Student Services Department at 937-259-6603. If new to the district, your student must be registered at the building based on your address before applying. These applications are on a first-come, first-served basis. Approval is based on space availability within the requested grade/school. 

    Exceptional Education & Student Services
    The District provides educational opportunities to students who are physically handicapped, mentally handicapped, sensory impaired, emotionally disabled, as well as, gifted. Parents interested in more information about special education services should contact Student Services @ 937-259-6603.
