To register a student, please go to the Mad River Local Schools District website. Click on the Parent OneView Access Tab and create an account. Once you create an account, you should be able to fill out all the necessary paperwork to get your child registered.
Please bring with you the following to register:
Original Birth Certificate
Two proofs of Residency - Lease Agreement or Mortgage Statement and a Bill
IEP or 504 Plan if your child has one
The following Forms:
If your child will need medication at school, please have the following form filled out by a doctor.
