Parents, it is very important that you return or mail back the impact aid survey that was either sent home or mailed to you. Mad River is gathering Federal Impact Aid surveys from families of federally connected students to assist the district in recouping lost tax revenue due to the presence of tax-exempt federal property, or increased expenditures due to the enrollment of federally connected children.
It is critical that all federally connected families participate as this funding helps us better support these families and maintain our level of services for Mad River Local Schools.
In order to collect this information, surveys were sent home beginning October 14, 2020 and must be returned by October 21, 2020.
The information obtained from this survey will be kept confidential and used for these reporting requirements only.
For in-person families: Please complete the brief survey and return it promptly to your child’s homeroom teacher by Wednesday, October 21, 2020.
For remote families: Please mail your survey back to the district using the self-addressed stamped envelope included with the survey mailed out to families and return it promptly to the district by Wednesday, October 21, 2020.
Should you have more than one child in the Mad River Local Schools, a survey form MUST be completed for EACH child. The Federal Government mandates a completed, signed, and dated form be submitted for each student currently enrolled in Mad River Local Schools regardless of where the parent(s)/guardian(s) are employed. If you are a Civilian/Private Contractor employed on or reporting to work on Federal Property, you must include your employer's address as listed on your W2.
